This policy MUST be signed at initial appointment.
Please try to arrive no-more than 5-10 minutes prior to your scheduled time so you receive your full appointment time ensuring the best look possible. We are a “By Appointment” studio, we arrive 15-20 minutes before appointments.
Please read the following policies, initial each one, and then sign your name at the bottom of the page to acknowledge that you agree to the following terms.
CANCELLATION POLICY
If you need to cancel an appointment, please contact us ASAP (24 hour notice) so we have the opportunity to offer your appointment to another client. You receive a confirmation prior to appointment which gives you the opportunity to change/ cancel. If less than 24 hours’ notice is given you will be charged a $25.00 cancellation fee. We have the right to refuse service to anyone.
NO-SHOW POLICY
If you do not show up for a scheduled appointment, you will be charged up to 50% of your scheduled service price.
After your first appointment your credit card information will be collected in case of infraction.
If you are late for an appointment, you will be seen as soon as possible, and for the length of time remaining to your appointment. If you are over 15 minutes late, your appointment will have to be rescheduled, and you will be charged a $30.00 fee.
I understand the terms of this form. I realize that I am financially responsible for any charges incurred from cancellations or no shows.
LASH RENTION POLICY
REFUND POLICY:
WE DO NOT OFFER REFUNDS. We are willing to fix/or correct any mistakes. We have the right to refuse service to anyone.
FULL-SET RETENTION POLICY:
If you’re receiving a FULL-SET of Lash Extensions, we GUARENTEE them 7 days before you should have any significant loss (50%+ of eyelashes gone on one or both eyes). If you have significant loss within those 7days we will fix them or remove them free of charge.
FILL RENTETION POLICY:
If you are receiving a FILL of your current Lash Extensions, we GUARENTEE them for 4 days before you should experience any significant loss (25%+ of eyelashes gone on one or both eyes). Integrity of the remaining lashes is not always clear, and they can fall out prematurely. If you have significant loss within those 4 days we will fix them free of charge.
ALLERGIC REACTION POLICY:
If your eyes are sensitive, please advise your lash artist beforehand.
If you should have an allergic reaction to the Lash Adhesive, Extension fibers, or under-eye pads we will remove the lashes free of charge. If you choose to continue with the extensions we will do a “patch test” in advance to make sure the reaction does not reoccur. YOU WILL HAVE TO PAY FOR A NEW SET (We will honor our ‘new client special’ price). An allergic reaction is not our fault, we cannot control your body’s reaction to the products we use. We will do our best to accommodate your needs. WE DO NOT OFFER REFUNDS.
